Lost And Stolen Passport USA

American Citizen Services (ACS) appointments are required for all regular services. To schedule an ACS appointment online, please click on Online Appointments. Before booking a passport appointment, we advise you to read the following prerequisites. Call (880) (2) 5566-2000 during business hours or (880) (2) 5566-2000 x 0 after hours and ask to talk to the duty officer if this is an emergency or if you need to make an urgent trip. American emergency services are accessible every day of the week, around-the-clock.

In the event that you have misplaced or had your passport stolen, it is crucial to promptly inform the local authorities and the U.S. Embassy. It is advisable to promptly apply for a replacement passport following the occurrence of the loss or theft.

The updated version of Form DS-11 Application for a U.S. Passport now mandates customers to provide a photocopy of their primary proof of citizenship, along with a certified copy from the United States. Please note that when filling out the form DS-11, you are currently unable to specify a “Country” under the “Emergency Contact” section. Kindly provide an emergency contact residing in the United States.

The main forms of evidence of primary citizenship for this initiative include U.S. birth certificates, naturalization certificates, citizenship certificates, and consular reports of birth abroad (CRBAs).

Application criteria

In order to prove your citizenship, you may provide any of the following documents, if available:

  1. A U.S. birth certificate
  2. A U.S. Consular Report of Birth Abroad
  3. A U.S. Certification of Birth
  4. A U.S. Certificate of Citizenship
  5. A U.S. Naturalization Certificate

Proof of Identity:

To confirm your identity, you may submit any of the following documents:

  • A valid passport from any country.
  • A valid U.S. driver’s license with a clearly visible color photograph.
  • A national identity card with a clearly visible color photograph.
  • A valid U.S. or state government identification card.
  • A U.S. military identification card.

Please complete the online passport application form (DS-11) and the statement for lost and stolen passports (DS-64) on the internet. After filling out the forms, make sure to print a copy of them to bring with you to your appointment. Please note that when filling out the “Emergency Contact” section on the DS-11 form, you cannot currently specify a “Country”. Instead, provide an emergency contact located in the United States. Please complete a police report to document the stolen passport.

Payment for the passport fees must be made in cash, either in United States Dollars or the equivalent amount in Bangladeshi Taka.

Situations Particular to Minors Under 16

Both parents must sign the application in person in front of the consular officer if the applicant is a child under 16 (formerly 14) and must appear in person at the time of application. One parent must provide a signed and notarized statement of consent (DS-3053) in order for the kid to be granted a U.S. passport if one parent is unavoidably unable to be present at the time of the interview.

The consent must be written in English and may be notarized by any notary public in the United States or at any embassy or consulate of the United States overseas. Additionally, you need to include a copy of the photo ID that you used to notarize the DS-3053 form. This page has all passport application forms.

If one parent has sole custody of the minor kid, that parent must show legal proof that the sole custody was granted by an appropriate court of jurisdiction.
